How to Detect and Govern AI Agents in Your Organization

A few months ago, a Director of Alignment at Meta's AI Safety Lab — someone whose literal job title is AI safety — gave an AI agent access to her email inbox. She told it explicitly: confirm before acting. Don't do anything without checking with me first.

The agent followed that instruction faithfully for weeks. Then she pointed it at her real inbox. The agent hit its memory limit mid-task, forgot the instruction, and kept going. She couldn't stop it from her phone. By the time she reached her computer, it had bulk-deleted her inbox. Her words: "I had to RUN to my Mac mini like I was defusing a bomb."

If the person whose career is built around preventing exactly this kind of failure couldn't stop it — what does that mean for your organization?

That story is not an outlier. It is a preview. And the reason it matters is not the tool that failed. It is what the tool was: an AI agent. Not a chatbot. Not a search assistant. Something fundamentally different — and something that is already running inside most organizations right now, without oversight, without governance, and without anyone in leadership knowing it is there.

Most Leaders Think They Have an AI Tool Problem. They Actually Have an AI Agent Problem.

Before we go further, the distinction matters.

An AI tool waits for you. You ask it a question, it answers, it stops. ChatGPT is a tool. Microsoft Copilot in chat mode is a tool. You are always in the loop. The risk is bounded: whatever data you put in may go somewhere it shouldn't. That is a real problem, but it is a visible one.

An AI agent is different in one critical way: it acts on your behalf. You give it a goal — research this vendor, schedule these meetings, process these invoices, monitor this inbox — and it figures out the steps, accesses the systems it needs, takes actions, and keeps going until the goal is complete. It does this around the clock. It does not check in unless you designed it to. And it operates under your identity, with your permissions, on your systems.

That autonomy is what makes agents powerful. It is also what makes them a different category of risk entirely. A tool creates a record of human decisions. An agent creates a record of machine decisions made on behalf of humans — and that record is almost never captured in a format your security team can use.

Gartner predicts that 40% of enterprise applications will be integrated with AI agents by the end of 2026, up from less than 5% in 2025. That is not a slow adoption curve. That is a transformation happening inside your organization right now, driven by individuals who are trying to do their jobs faster and who have no idea they are creating a governance gap in the process.

You Probably Have Agents Running Right Now and Don't Know It

This is the thing that surprises most leaders when we first start working with them. The assumption is that AI agents are a future problem — something to govern before it gets out of hand. The reality, in almost every organization we engage with, is that agents are already running. They were built by a project manager trying to automate a reporting workflow. A sales lead who wanted to stop spending two hours a day on prospect research. An operations coordinator who connected an AI tool to the company calendar and told it to handle scheduling.

None of these people did anything wrong. They were doing their jobs. They had no idea they were creating compliance exposure, access control gaps, or audit trail failures. They just clicked through a setup wizard and got back to work.

This is not an IT failure. It is a structural consequence of how agents get built. Microsoft Copilot Studio — bundled into many Microsoft 365 subscriptions — allows any employee to build a fully functional agent in under an hour with no technical background. Google, Salesforce, and dozens of standalone platforms offer the same. The barrier to creating an agent that connects to your email, CRM, and file system is lower than the barrier to requesting new software through your IT department.

Only one in five companies has a mature model for governing autonomous AI agents. Which means four out of five organizations — including almost certainly yours — are deploying agents without the visibility, controls, or accountability structures to manage what those agents are actually doing.

How to Find What Is Already Running

The inventory exercise is the most important first step in any AI governance program, and it is simpler than most organizations expect.

Ask your team directly — and make it safe to answer honestly. Send a no-judgment message: what AI tools are you using for work, what tasks do you use them for, and have you connected any AI tools to your work email, calendar, or files? Frame it as a visibility exercise, not an audit. The tone of the request determines the accuracy of the responses. If people are afraid of the question, you will get compliance-signaling, not truth.

Review your platform environments. Microsoft 365 admins can check Copilot Studio for agent deployments and Azure Active Directory for third-party app connections. Google Workspace admins can review connected applications. Most of the agents your team has built will appear in one of these places — if someone is looking.

Check what credentials your agents are running on. This is the single highest-risk finding in most environments. When an employee builds an agent and shares it with their team, the agent typically continues running on the builder's personal credentials — carrying the builder's full access to every system they can reach. A junior employee using a shared agent may be accessing executive-level financial data, acquisition strategy documents, or personnel records without anyone realizing it. Ask IT to identify every automated process running on personal employee credentials. Each one is a priority remediation item.

Repeat this process quarterly. Agents will be built faster than your review cycle. The goal is not a perfect permanent inventory — it is a regular visibility practice that catches new deployments before they create serious exposure.

Three Risk Patterns That Show Up in Almost Every Organization

Once you know what agents are running, the next question is which ones carry real risk. In our experience working with organizations across industries, three patterns appear consistently — and all three are present in most environments long before anyone in leadership knows to look for them.

Pattern one: the shared agent. An employee builds a useful agent and shares it with their team. The agent keeps running on the builder's credentials. Twelve people are now operating with one person's access level. When something goes wrong, the audit log shows one name. There is no way to reconstruct who did what. In regulated environments — particularly for federal contractors — this is not just a security problem. It is a documentation failure and potentially a compliance violation.

Pattern two: the hijacked agent. This is the risk security researchers are most concerned about right now, and it has no complete fix. When an AI agent reads external content — a website, an email, a vendor document, a calendar invite — it processes that content the same way it processes instructions from its user. If an attacker embeds hidden instructions inside that content, invisible to a human but readable by the AI, the agent will follow them. It might be directed to forward your CRM contacts to an external address, pull files from your local system, or take actions your authorized user never requested. The user sees a normal response. No alert fires. This class of attack has been demonstrated against production AI platforms. It requires no special access and no user error — only the ability to place content somewhere the agent will read it.

Pattern three: approval fatigue. The accepted answer to agent risk is human-in-the-loop oversight: before significant actions, the agent asks for approval. This works until the human stops reading the requests. An agent generating dozens of approval requests per day trains the approver to click through efficiently. By Wednesday afternoon, when the agent asks to forward executive communications to an external address, the approver clicks approve — not because they reviewed it, but because clicking approve has become the automatic response. The control exists on paper. The human is no longer actually in the loop.

The Compliance Layer Most Organizations Are Missing

Here is where the conversation changes for federal contractors, regulated industries, and anyone whose clients ask about data handling.

AI agent governance is not just a security conversation. It is a compliance conversation. And for many organizations, the compliance implications are more urgent than the security ones — because the obligation exists whether or not an incident has occurred.

If your organization handles federal project data, that information may be classified as Controlled Unclassified Information under DFARS 252.204-7012. Pasting CUI into an unapproved AI tool is a potential compliance violation regardless of intent, regardless of whether the data was misused, and regardless of whether any harm occurred. One employee, one paste, one contract at risk.

If your organization holds CMMC certification, your system security plan must document and control how sensitive data is handled across all systems and workflows. AI tools are systems. AI agents are components of your workflow. An undisclosed agent touching federal project data is a gap in your CMMC program — not just a security concern.

If your employees hold security clearances, using an unapproved AI tool to process work-related information may constitute a reportable personnel security incident. The standard is not whether harm occurred. It is whether proper handling procedures were followed.

If you have cyber insurance, your carrier is almost certainly asking about AI usage now. Most organizations answer no — incorrectly — because no one did the inventory. That is a coverage gap waiting to become a claim denial.

These are not future risks. They are current obligations that most AI governance conversations are not addressing.

What Good Governance Actually Looks Like — and Why It Is Not a Bureaucracy Problem

Companies that implemented AI governance programs pushed 12 times more AI projects into production than those that did not. This is the data point worth sitting with, because the assumption in most organizations is that governance is what slows adoption down. The evidence says the opposite. Governance is what allows adoption to scale — because it builds the organizational confidence to deploy agents in higher-value scenarios, rather than treating every agent as an unmanaged risk.

The minimum viable governance program for most organizations has three components.

An agent approval process. Before any agent goes into production, someone with appropriate authority answers six questions in writing: What is the agent's defined task? What data can it access? Does it have access to private data, external content, and communication capability simultaneously — and if so, which can be scoped down? What credentials is it running on? Can you reconstruct what it does from the logs? How do you stop it if something goes wrong? This review should take under an hour for most agents. The documentation is the point — it is evidence that your organization is actively managing the risk.

An AI acceptable use policy. Your existing acceptable use policy almost certainly does not address AI agents. You need either a standalone policy or a substantive addendum that defines what tools are approved and how a tool earns approval; what data categories may never go into an unapproved tool; who can build agents and what is required before deployment; and what employees should do if they think they made a mistake. That last item matters more than most policy writers realize. If reporting a mistake triggers discipline, you will get silence. If it is safe to self-report in good faith, you will get accurate information — which is the only thing that lets you actually manage the problem.

Logging that can be used in an investigation. Most AI platforms do not produce logs in a format a security team can act on. Before deploying any agent in a production environment, verify that your logging infrastructure captures what that agent does, on whose behalf, and when. For regulated organizations, this is not optional. It is the difference between a manageable incident and an unmanageable one.

The Accountability Gap — Who Actually Owns This

Here is the pattern we see in almost every organization we work with. IT assumes the business team is responsible for the tools they chose to use. Business teams assume IT reviewed and approved anything in production. Legal assumes procurement vetted the vendors. Leadership assumes everyone is using approved tools. Everyone's assumption is reasonable. Everyone's assumption is wrong.

The gap between four reasonable assumptions is where incidents live — not because anyone failed, but because nobody owns the question.

This is not fixable with a better framework or a more thorough policy. It is fixable with a decision. Someone in leadership has to decide — before an incident, before a contract audit, before an insurer asks a question you cannot answer — that your organization is going to manage its AI agent environment with the same deliberateness it applies to every other category of operational risk.

Organizations that make that decision now have a meaningful advantage. The regulations are still catching up. The CMMC assessors are still developing what AI coverage looks like. The contracting officers are still formulating the questions they will eventually ask. The organizations that can answer those questions with documented evidence — rather than assurance — will be in a fundamentally different position.

Four Things You Can Do Before Friday

You do not need a budget or an IT project to move meaningfully on this.

Send the visibility email today. Ask your team what AI tools they are using, what they use them for, and whether any are connected to company systems. No judgment. You want the truth, not the version they think you want to hear.

Read the data terms of your most-used AI tool. Search the privacy policy for "training data" and "improve our services." See what your team agreed to when they clicked accept. Flag anything that creates exposure for legal review.

Find one agent running on a personal login. Ask IT. If one exists, moving it to a dedicated service account with scoped permissions is your highest-priority remediation item. One change. Closes one of the most common access-control gaps immediately.

Name an owner. AI agent governance needs one person or function accountable for it — maintaining the approved tool list, running the agent review process, reporting status to leadership. Not a committee. Not shared responsibility. One name. Until that exists, the program does not exist.

The agents are already inside your organization. The question is not whether to prepare. It is whether you can see what is already running before it becomes your problem.
